Position Summary:
Under the general directions of the Clinical Supervisor Case Manager, the MFTI, PCC, ASW, MSW, Trainees, and Psychologist Interns is responsible for providing support to members of the Outpatient & Day Rehab program within the Creative Wellness Center.

  1. Perform Case Management and stage-based dual diagnosis treatment services including integrated assessment, relapse prevention, crisis intervention, and social skills training.
  2. Clinical responsibilities include: formulating individual assessment & treatment plans for client caseload and providing case management services; assisting in clinical staff meetings; participating in individual supervision and group supervision; and acting as a liaison with other mental health agencies.
  3. Utilize Solution Focused Brief therapy
  4. Conduct outreach to engage clients and enlist family support
  5. Provide psychoeducation, skills-building, and linkages to community supports
  6. Maintain appropriate Medi-Cal clinical documentation.
  7. Performs duties of mental health and substance abuse counselor in Day Rehab program.  As a member of the clinical team, coordinates treatment and services for adults who have a psychiatric disability or who have co-occurring disorders, provides ongoing assessment, counseling and intervention; establishes linkages and acts as a service broker to meet clients' comprehensive needs.
  8. Provides crisis management services as needed.
  9. Responsible to plan and facilitate a variety of treatment groups in the Day Rehab program.  
  10. Conducts intake interviews as assigned that include: educational and community liaison functions; clinical assessments; and written documentation.
  11. Documentation responsibilities include writing weekly progress notes.  It is a requirement of the position that client charts be maintained in a timely way according to regulatory standards.
  12. Requirements and specific on-site program operations as assigned.
  13. Participate in planning and conducting occasional outings and holiday events.
  14. Perform office management tasks as assigned.
  15. Other duties assigned by Clinical Supervisor Case Manager.
Minimum Qualifications:
  • MFTI, PCC, ASW, MSW, Trainees, Psychologist Interns
  • Spanish speaking/bilingual persons and people with lived experience encouraged to apply.
  • Must demonstrate an ability to work both independently and as a member of the team.
  • The ability to prioritize effectively on competing demands on time.
  • Must be able to maintain flexible work schedule in accordance with client and program needs.
  • Solid written skills to meet required standards of documentation.
  • Must be able to meet Immigration Reform Act of 1986 requirements.
  • Must have valid California Drivers License and a driving record acceptable by the Agency’s insurance company.
  • Must be able to pass physical examination.
  • One year experience providing co-occurring mental health and substance abuse treatment to priority populations.  
  • Required license or certification: MFTI PCC, ASW, MSW, Trainees, Psychologist interns Registration with CA BBS

Job Type:
  • Part-time 15.5 hours per week

  • Master’s Degree
  • Must have valid California MFT Intern registration and be in good standing.
  • One year experience as a Marriage and Family Therapist Intern
  • Casa Ubuntu will provide supervision to sign off on hours
